			<content:encoded><![CDATA[<p>Dependency on drugs can affect anyone and affects a lot more people than the statistics tell us. When I talk about drugs I mean all drugs, legal and illegal. This could be cocaine, cannabis, alcohol, tobacco and even prescription drugs. We could even be talking about caffeine here. Strangely it&#8217;s the legal ones that cause the most problems in society, alcohol and tobacco being the main offenders. You only have to read the UK&#8217;s top expert Professor Nuts reports to see this. But then you only have to drive around most towns late on a Friday or Saturday to see for yourself the problems it causes. Or ask any nurse or doctor. They see thousands of people suffering from heath problems caused by alcohol and tobacco.</p>
<p>Alcohol is one of the worst drugs out there but because its legal people think it&#8217;s ok to take it. But if your alcohol use has a negative effect on you, such as wanting to take more, then you are probably suffering from drug dependency. The symptoms are easy to see. If you can&#8217;t go a few days without alcohol then you have a dependency on the drug. You could also be dependant if you only have one pint a week on a Sunday for instance. If you miss that Sunday and suffer from withdrawal symptoms then you have a once a week dependency on the drug. The withdrawal symptoms are symptoms such as sweating, shaking and feeling sick. If you feel like this when you have missed your daily/week or monthly dose of your chosen drug then you are definitely dependent on it. If you feel irritable, depressed or tired then you have a psychological dependency. Either way it is a problem and could lead to much more serious issues later in life, so getting yourself off the drug should be top of your priority.</p>
<p>The first step to recovery of any drug is acceptance. Until you admit to yourself that you have a problem only then can you get help. Detoxification and psychotherapy are some of the options used to treat drug dependency. The main goal is complete abstinence and to get this far will not be easy. Support from family and friends is very important and if your friends are drug takers then you may need to distance yourself from these people and find new friends. Opening up and talking about your problems is not so difficult if you are talking to people who are or have gone through the same issues.</p>

But you could research some fascinating subject on the internet, like the mind-numbing hugeness of the universe. That&#8217;ll get you going!</p>
<p>8. Breathe</p>
<p>Stop, relax and be aware of your breathing from the stomach.</p>
<p>9. Let go of things from the past</p>
<p>Still thinking of that time those boys beat you up years ago? Me, too. But don&#8217;t! It didn&#8217;t happen in a very real sense because the physical wounds have healed and keeping the mental ones hurting makes as much sense as the beating did. Your memory is unreliable, don&#8217;t feed it with brain time. Concentrate on the present moment. The present moment is good.</p>

<p>In the past 50 years, many confrontational psychodynamic approaches became popular and almost the expectation of substance abuse and addiction treatment. There was the idea that substance abuse was a symptom of an underlying disorder (Weegmann, 2002). Once the disorder was treated, the secondary substance abuse would clear up on its own. As Weegmann discussed, research has shown that highly confrontational methods often instilled an ever deeper sense of resistance in the client. The percentage of successful outcomes within the field of addiction have been tossed around and range a great deal, however they have always seemed pretty dismal in comparison to the length and cost of many treatment programs. A new approach is necessary and an approach that is more accessible to those in need of it would be highly valuable in the field.<br />
<strong><br />
Addiction and Substance Abuse</strong></p>
<p>According to Miller (1998), addiction can be described within two conditions: (1) a behavior that persists even though there is apparent risk or harm to oneself or others and (2) to an outside observer the individual demonstrates diminished, but retrievable, capacity for self-regulation of the behavior. Miller also discusses &#8220;symptoms&#8221; of addiction which include: giving inordinate priority to the addictive behavior; avoidance of situations where the behavior is inaccessible; and giving a high priority to the behavior after a period of abstinence, among others. Miller suggests that the problem of addiction is simply competing motivations. Motivation for the behavior currently outweighs the harm and perceived value of available alternatives to that behavior.</p>
<p>So how does an addicted individual escape addiction? Miller (1998) argues that there can be a sudden shift in how the person perceives the pros and cons of their behavior. This is often seen over time as the suffering directly attached to the addiction increases and the ratio of pros and cons shifts. Through motivational interviewing, Miller suggests that therapists and other helping professionals can lend clients another perspective, a mirror image of themselves, so as to increase the client&#8217;s conception of the consequences and saliency of their behavior.<br />
<strong><br />
Adolescence and Young Adulthood</strong></p>
<p>Within efforts to utilize motivational interviewing within adolescence, motivational interviewing offers several advantages over other treatment options. According to one article (Tevyaw &amp; Monti, 2004), motivational enhancement and other brief interventions have the highest effect sizes among all treatments for alcohol abuse and dependence and evidence similar efficacy rates. Motivational techniques also provide a more flexible continuum of treatment ranging from abstinence to harm reduction within cases where abstinence is not necessary, which is often the case with adolescents with short histories of substance abuse (Tevyaw &amp; Monti, 2004). Adolescents also often end up in a treatment setting due to a decision made by an authority figure such as their parents or the justice system and concerns are often not shared by the adolescent. They are often characterized by resistance to change and a lack of understanding of the consequences of their behavior. Motivational techniques provide clinicians with an attractive method to increase rapport and commitment to change from clients that have been deemed as resistant in the past (Tevyaw &amp; Monti, 2004).</p>
<p>It is often not necessary for adolescents to admit to or acknowledge having substance abuse problems in order for them to benefit from a motivational approach. Motivational techniques also do not rely on diagnostic labels allowing for the client to benefit at any level of readiness to change. It can raise awareness during the pre-contemplation stage, can help in decision-making during contemplation, can enhance a person&#8217;s resolution to change during the action and maintenance stages, and after a relapse it can provide an opportunity for reassessment (Ford, Oliver &amp; Whitehead, 2006).</p>
<p>Within young adulthood, motivational interviewing provides similar advantages over other treatment options, however the ability to utilize brief interventions becomes incredibly valuable at this stage. Young adults are at a much higher risk of the drastic effects of substance abuse due to their distance from home and authority. As Monti and Tevyaw (2004) point out, young adults in the 18 to 25 age range consistently engage in high rates of risky behavior such as heavy substance abuse and unprotected sex. These individuals are less likely to be confronted by family due to their distance from them or by friends due to the general acceptance of this behavior during this life stage. It is therefore important to utilize brief motivational interventions within areas where young adults present themselves such as the emergency department, college counseling and health centers, and within employee assistance programs. Motivational interviewing provides a simple, trainable technique that can be used in any of these settings in a brief period of time.<br />
<strong><br />
Outcomes</strong></p>
<p>Each study agreed that motivational interviewing provided at the very least a comparably effective option for substance abuse treatment. Within one study (Schneider, Case &amp; Kohn, 2000), clients who were placed with counselors practicing motivational interviewing showed higher satisfaction rates with their experience. In another study among heavy drinkers on college campuses, brief interventions of motivational interviewing results in lower self-reported rates of alcohol consumption and lower blood alcohol levels than individuals who simply received feedback (Monti &amp; Tevyaw, 2004). At a 6-month follow-up, those who had received the motivational interview showed a significantly lower rate of drinking and driving, traffic violations, and other alcohol-related problems or injuries (Tevyaw &amp; Monti, 2004).</p>
<p><strong>Conclusion</strong></p>
<p>Motivational interviewing is a simple, highly trainable, and effective technique that approaches addiction in a unique way. It provides a theoretical framework that falls more in line with social work values and ethics than many others. It allows for self-determination, respect, and validation of the client&#8217;s strengths. An emphasis is on giving the client something new. Especially within substance abuse, the client has heard from multiple authorities and trusted family and friends that their behavior needs to change. The framework also allows for brief interventions that can take place in almost any setting and do not require a professional therapist to initiate. This allows for more manageable, accessible, and cost-effective treatment.</p>

			<content:encoded><![CDATA[<p>Employer drug testing has become an important safety issue in the workplace not only for employees, but for human resources and safety professionals as well. It is estimated that over 98% of all the Fortune 500 companies conduct drug testing. The purpose of employer drug testing is to lessen the impact from drug abuse in the workplace. This includes tardiness, absenteeism, turnover, attitude problems, theft, deceased productivity, crime and violence. The US Department of Labor has estimated that drug use in the workplace costs employers anywhere from $75 to $100 billion dollars annually in lost time, accidents, health care and workers compensation costs. Sixty-five percent of all accidents on the job are related to drug or alcohol abuse, and substance abusers utilize sixteen times as many health care benefits and are six times more likely to file workers compensation claims then non-drug abusers.</p>
<p>Drug experts are debating on whether drug abuse has fallen, or drug abusers simply avoid employers that test and instead apply at companies that do not test. Either way, most human resource and safety professionals have found drug testing to be a valuable and cost-effective risk management tool.</p>
<p>When implementing a drug-testing program, policies and procedures should be established. Pre-employment drug testing is the most common type of testing program. Courts have consistently upheld the legality of requiring a pre-employment drug testing as a condition of employment.</p>
<p>However, if a firm plans to conduct post-hiring drug testing for current employees, then the employer should include training and education for supervisors and employees, as well as guidelines for discipline in the event of a positive test.</p>
<p>Post-employment drug testing includes random testing for safety sensitive positions, individualized suspicion testing, post accident testing, and testing that is legally required in certain industries, such as Department of Transportation (DOT) requirements concerning truck drivers. Each of these types of testing is legally sensitive, and an employer should have a program in place before starting.</p>
<p>Most drug testing is done by sending an applicant to a collection site, where a urine sample is obtained and sent to a certified laboratory for analysis. Negative results are normally available within 24 hours. There are also instant drug testing and alcohol testing kits on the market that are available for employers to use. These are similar to home pregnancy tests and require the employer to collect a urine sample. These drug tests and alcohol tests are considered accurate for immediate screening at the convenience of the employer.</p>
<p>Most employers utilize a standard five-panel test of &#8220;street drugs,&#8221; consisting of marijuana (THC), cocaine, PCP, opiates (such as codeine and morphine) and amphetamines (including methamphetamine). Some employers use a ten-panel test, which includes prescription drugs that are legal to possess and use. Employers can also test for blood alcohol levels through alcohol testing kits.</p>
<p>Although each drug and person is different, most drugs will stay in the system for 2-4 days. For chronic users of certain drugs, such a marijuana or PCP, results can be detected for up to 14 days, and sometimes much longer. Sedatives, such as Valium, may stay in the system for up to 30 days. When the more expensive hair testing method is used, drugs can be detected for a 90-day period. To avoid the complications from &#8220;second hand&#8221; marijuana smoke, most labs will set a higher threshold before reporting THC in the system.</p>
<p>Testing labs have extensive procedures to re-confirm a positive test before reporting it. Most drug testing programs also utilize the services of an independent physician called a Medical Review Officer (MRO) to review all test results. In the case of a positive result, the MRO will normally contact the subject to determine if there is a medical explanation for the positive results. For example, eating poppy seeds before a test can result in a false positive for opiates. However, an MRO also knows that poppy seeds cannot cause certain levels of opiates, and certain additional testing can eliminate that.</p>
<p>There can also be tests that are &#8220;negative&#8217; but show an abnormal result, such as a &#8220;low creatine level,&#8221; which can indicate an applicant attempted to dilute the sample by the excessive drinking of water or some other form of alteration. That is also a result that a MRO would examine.</p>
<p>Employers who conduct employer drug testing will find that a drug testing program will eliminate people with a drug abuse problem. Drug tests generally cost in the $50-$70.00 range, including collection of the sample, laboratory analysis, services of a Medial Review Officer, and communications of the results in the manner most convenient to the employer. Compared to the cost of even one employee with a substance abuse problem, most firms find eliminating the problem in the first place is well worth the time and money involved in an employer drug testing program.</p>

<p>Drug use disorders and other mental illnesses are developmental disorders. That means they often begin in the teen years or even younger—periods when the brain experiences dramatic developmental changes. Early exposure to drugs of abuse may change the brain in ways that increase the risk for mental disorders. Also, early symptoms of a mental disorder may indicate an increased risk for later drug use.</p>
<p><strong>How Are These Comorbid Conditions Diagnosed and Treated?</strong></p>
<p>The high rate of comorbidity between drug use disorders and other mental illnesses calls for a comprehensive approach that identifies and evaluates both. For example, evidence suggests that bupropion (trade names: Wellbutrin, Zyban), approved for treating depression and nicotine dependence, might also help reduce craving and use of the drug methamphetamine. More research is needed, however, to better understand how these medications work, particularly when combined in patients with comorbidities.</p>
